Tournament check-in will be from 6:00 to 8:30 pm on Friday, September 18, 2009 at the Marriott Courtyard. Phone 513-733-4334. All teams must register on Friday night. Thank you.

Referee Assignor Announced

Registration at Marriott Courtyard, Blue Ash,Ohio

You must register your team on Friday, September 18th from 6:00 to 8:30 at the Marriott Courtyard located at 4625 Lake Forest Drive in Blue Ash, Ohio 45242. Local phone 513-733-4334.

You will need your player cards, two copies of your roster for us to keep, and medical releases. Medical releases must be notarized for all teams outside of Ohio South and Travel permits are mandatory for teams outside of Ohio South. Ohio South teams may present either medical releases or their player membership forms. For Guest players, you will need a state approved card plus an approved guest player form. There will be a representative from Ohio South if teams need to guest card a player with a picture, birth certificate, membership form and medical release. Player cards will be available for sale.

WHAT TO BRING TO REGISTRATION
June 01, 2009
Registration is at the Marriott Courtyard, 4625 Lake Forest Drive, Blue Ash, Ohio 45242 from 6:00 to 8:30 pm. All teams must register Friday night. We will not begin until 6:00 pm therefore, we would appreciate it if you do not come early so that we can get set up to process your registration quickly.

1. Player Cards and Coaches cards

2. Notarized Medical Release Forms - these will be returned to you at check-in. Only Ohio South teams may use their membership forms in place of notarized medical releases.

3. Two copies of your approved league roster for the tournament to keep.

4. Guest Players -if you have a guest player you will need their card, approved guest player form from your state association and a medical release. If the player is not carded, please bring a small photo, birth certificate copy (no hospital records accepted), and medical release. You may buy a card at registration and the card will only be good for the American Cup Soccer Tournament.

5. Travel permit- all teams traveling outside of Ohio South need to present a travel permit.

6. Contact form- This form is on our website. Please make sure that we have all the contact information for this weekend should we have any unforseen changes with fields and/or scheduling. We suggest you check the website during the weekend for any updates.
